By choosing to receive services at Uneka, you acknowledge and agree to the following terms:

Acknowledgment:

I voluntarily choose to receive the esthetic treatments provided by the licensed estheticians at Uneka. I confirm that I have provided a complete and accurate medical history, including any known allergies, medications, and previous adverse reactions to similar treatments. I understand that it is my responsibility to inform the esthetician of any changes in my medical history.

I understand that the treatments provided are not medical treatments and do not substitute for medical care. All treatments are performed according to the best professional judgment of the estheticians at Uneka, but results can vary from client to client.

Release:

I am aware that certain adverse effects may occur as a result of the treatments, including but not limited to redness, swelling, irritation, or allergic reactions. I acknowledge that I have been informed about the potential risks and benefits of the treatments provided.

I agree to follow the aftercare instructions given to me by my esthetician and to seek medical advice if I experience any unusual symptoms following the treatment.

By receiving treatments at Uneka, I release and discharge the estheticians and Uneka from any and all claims, demands, or causes of action that I may have now or in the future, arising from the treatments provided. This includes but is not limited to claims for bodily injury, property damage, or any other adverse effects resulting from the treatments.

Consent:

I have read and fully understand the information contained in this acknowledgment and release statement. By proceeding with the treatments, I consent to the terms outlined above and assume full responsibility for any adverse effects or undesirable results.
